Alternating case flips every letter between lowercase and uppercase in turn, starting with lowercase, so hello world comes out as hElLo WoRlD.
Only alphabetic characters take part. Spaces, punctuation and digits pass through untouched and do not advance the toggle, so the rhythm carries on across word boundaries rather than restarting at each new word.
It reads as mockery. Somewhere along the way this pattern became online shorthand for repeating something sarcastically, the written version of a sing-song voice, and that is what most people are reaching for when they generate it.
The rest of the time it is decorative: usernames, captions, and anywhere a plain string is meant to look deliberately unserious.
For actual emphasis rather than mockery, the conventional options are better in every way. Bold and italic carry meaning that alternating case does not, and all caps is the older signal for volume.
